Abstract

Marshall syndrome (PFAPA syndrome) is the most common autoin{ ammatory syndrome of childhood
and is characterized by periodic episodes of fever, pharyngitis, aphthous stomatitis, and cervical lymphadenitis. In recent
years, increasing attention has been paid to the role of chronic tonsillitis and immunological disorders in the pathogenesis
of this syndrome. The aim of the present review is to analyze current data on the therapeutic management of PFAPA
syndrome in children, with particular emphasis on the immunopathogenetic rationale for tonsillectomy. Analysis of the
literature indicates a leading role of cytokine imbalance, primarily interleukin-1β, and a high eff ectiveness of surgical
treatment in carefully selected patients.
