Abstract

Patulous Eustachian Tube syndrome (PET) is a rare but clinically signi cant disorder characterized by a disruption of the normal closure mechanism of the eustachian tube, leading to pronounced auditory and vestibular disturbances that substantially reduce patients’ quality of life. Despite growing interest in this condition, questions
regarding its etiopathogenesis, objective diagnosis, and optimal management remain controversial. The aim of this study is to systematize current data on the etiopathogenetic mechanisms of PES, analyze its clinical manifestations,
diagnostic methods, and contemporary therapeutic approaches based on the latest scienti c literature. This article presents information on the role of anatomical, neuromuscular, metabolic, and hormonal factors in the development of
PES. Clinical manifestations of the disorder are discussed, including autophony, a sensation of ear fullness, and uctuating hearing. Particular attention is given to instrumental diagnostic methods, such as tympanometry, audiometry, endoscopic
examination of the nasopharynx, and imaging techniques. A comparative evaluation of diagnostic and therapeutic approaches is provided, allowing for the formulation of an optimal management strategy for patients with PES.
